Terms and conditions of entry for $50 Amazon gift card: For every 50 survey submissions, one winner will be randomly selected to win a $50 Amazon Gift Card. Only one entry per person. Survey submissions must be complete. Your computer’s IP address will be recorded on submission and duplicate entries will be voided. A valid email address is required for entry. Selected winner’s will be drawn at random and notified by the email provided in the survey form. Winner’s will have fifteen days to respond to a notification email to accept the $50 Amazon gift card, after which time the reward will be considered void. —–>
